At the Centre for Advanced Rehabilitation (CARe), our AHPC-registered hand therapists provide conservative rehabilitation for triangular fibrocartilage complex (TFCC) injuries and wrist sprains, from splinting through to a graded return to your normal activities.
TFCC injury and wrist sprain treatment at CARe
The triangular fibrocartilage complex (TFCC) is a cushion of cartilage and ligaments on the pinky-finger side of the wrist that stabilises the joint and absorbs load when you grip, push or twist. Injuries here, along with wrist ligament sprains more broadly, can follow a fall onto an outstretched hand, a twisting injury, or repeated strain from sports and manual work. At the Centre for Advanced Rehabilitation (CARe), TFCC and wrist sprain rehabilitation is led by the AHPC-registered occupational therapists in our hand therapy service, part of a clinic team of physiotherapists and occupational therapists. Care is conservative. We do not perform surgery or injections, and for many people with mild to moderate injuries, conservative management is a reasonable place to start.
How we approach TFCC injuries and wrist sprains
Assessment first
Your therapist will take a detailed history (how the injury happened, which movements provoke pain, and how it affects grip and daily tasks) and use clinical tests to assess the stability and range of your wrist. Because wrist pain can come from several structures, the assessment also considers the forearm, hand and grip pattern as a whole.
Custom splinting and wrist support
A period of splinting or bracing can protect the wrist while it settles, particularly in the early stages after injury. Our therapists fabricate custom splints fitted to your wrist and advise you on when and how long to wear them as you progress.
Graded exercise and wrist stability
Once acute symptoms settle, a graded programme of range-of-motion, strengthening and stability exercises helps restore confidence in the wrist, progressed at a pace that matches how your symptoms respond.
Ergonomics and load management
Repeated gripping, twisting or weight-bearing through the wrist can aggravate a TFCC injury. An ergonomics consultation looks at your work setup, tool use and daily habits, with practical changes to reduce load on the wrist while it recovers.
Pain-relief modalities where appropriate
Selected physical agent modalities, such as therapeutic ultrasound or heat therapy, may be used to support symptom relief as part of your wider plan.
Rehabilitation after wrist surgery
If you have had, or go on to have, surgery for a TFCC tear or wrist ligament repair, our hand therapists provide post-surgical rehabilitation (splinting, scar management and graded movement) following your surgeon's protocol.

What to expect at your appointment
Your first session includes a full assessment of your wrist and hand, a discussion of your work, sport and daily activities, and an agreed treatment plan. If you have imaging results or a specialist letter, bring them along. They are helpful, but not required to start.
Your home programme
You will receive clear guidance on splint wear, home exercises and activity adjustments so treatment continues between sessions, with your programme reviewed and progressed at each visit.

Frequently asked questions
Can a TFCC injury heal without surgery?
Many people with mild to moderate injuries improve with conservative treatment such as splinting, graded exercise and load management. Not every injury responds, and outcomes vary. If your symptoms are severe or not improving, your therapist will advise you to see a doctor or hand specialist.
Do I need an MRI before starting treatment?
No. You can book an assessment directly, and imaging is not required to begin conservative treatment. If you have already had an MRI or seen a specialist, bring the results along, as they help your therapist understand your injury.
When can I get back to sport or gym training?
This depends on how your wrist responds to treatment. Your therapist will guide a graded return to load-bearing activity, sport or weight training, adjusting the pace based on your stability and symptoms rather than a fixed timeline.
